1)What information I collect
Information is collected about you when you contact me to use my service and upon your first visit for treatment. Upon signing the consultation form on your first treatment, you are agreeing that I will keep your details confidential and abide by GDPR guidelines. You also agree that I can contact you via telephone, text message or email regarding your appointments and treatments with me.
2) Data I collect includes:
• Name, address
• Date of birth
• Address telephone number
• Email address
• Occupation
• Lifestyle
• Health and medical history
I only collect the following data (through positive opt-in) through my website when an enquiry is made; full name, email, contact number. No other data is collected through the website.
3) How I use information and how it will be stored
The data collected will be used to provide effective treatments and ongoing treatment. It will also allow me to gauge whether treatment will be suitable and beneficial to you as there are some contraindications that may affect your treatment with me.
I use the information provided to:
• Confirm and remind you of appointment
• Contact you directly in relation to appointment
• Gain an understanding of your body and its’ needs
• Assess whether treatment will be suitable and beneficial to you
• Refer you to another health or wellbeing professional with your consent
The information you supply on the online enquiry form has no public exposure and will be sent directly to my one and only business email address which only I have access to.
The initial consultation forms and treatment notes are securely stored in a locked cabinet within the treatment room.
4) Who I share it with
Information will not be shared with any third party under any circumstances unless you have explicitly given consent to do so. If required to by law I may need to disclose your details – to the police, regulatory bodies, or legal advisors. This will only be done in compliance with UK Law.
5) How long will the data be kept?
Your data will be securely held for 7 years after the date of your last treatment. In the case of minors’ records, these will be kept for 7 years after they reach the age of 18.
6) Your rights under Data Protection legislation
Under the Data Protection law, you have rights including:
Your right of access – You have the right to ask me for copies of your personal information.
Your right to rectification – You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure – You have the right to ask me to erase your personal information in certain circumstances.
Your right to restriction of processing– You have the right to ask me to restrict the processing of your personal information in certain circumstances.
Your right to object to processing – You have the right to object to the processing of your personal information in certain circumstances
.Your right to data portability – You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.

As mandated by law I will not charge a fee to process these requests, however if your request is considered to be repetitive, wholly unfounded and/or excessive, I am entitled to charge a reasonable administration fee
